Bridges to Health (B2H)

The Bridges to Health Program provides supportive community based services to children who suffer from emotional, developmental, and medical disabilities as well as their caregivers. B2H promotes healthy supportive relationships, quality education and a healthy work ethic.

Children must meet the eligibility requirements, to be determined by Local Department of Social Services (LDSS) or Office of Child and Family Services (OCFS), and be in a Foster Care placement prior to being enrolled in the B2H program.

The Children’s Home is responsible for providing direct services. These services include the following:

  • Skill Building
  •  Prevocational Services
  •  Supported Employment Services
  •  Special Needs Community Advocacy and Support
  •  Planned Respite Services in the community
  •  Family Caregiver Support Services
  •  Crisis Avoidance Management and Training
  •  Immediate Crisis Response Services
  •  Intensive In-Home Support and Services
